Can you use an HSA or FSA for NAD+ injections? What to check
HSA and FSA use for NAD+ injections depends on a prescription and a diagnosed condition. What the IRS and NAD+ programs say, checked 5 Nov 2026, plus cash prices.
By Julian Caraulani · Updated November 5, 2026
Maybe, but it is not automatic. The IRS counts a prescribed drug as a medical expense, and says expenses "merely beneficial to general health" do not count (IRS Publication 502). NAD+ programs are cash-pay, with plans from $89 a month. Your plan administrator makes the final call.
Compounded NAD+ is not FDA-approved. It sits in FDA's Category 1 list of bulk substances under evaluation (FDA category list). See the peptide legal status tracker.
What does the IRS say?
Publication 502 sets the test for HSA and FSA spending on drugs, as read on 5 Nov 2026:
- "A prescribed drug is one that requires a prescription by a doctor for its use by an individual."
- "Except for insulin, you can’t include in medical expenses amounts you pay for a drug that isn’t prescribed."
- Medical expenses "don’t include expenses that are merely beneficial to general health, such as vitamins or a vacation."
(IRS Publication 502.) We found no IRS statement that names NAD+. The rule turns on two things: a clinician prescribes it, and it treats a diagnosed condition. NAD+ taken for energy or longevity is the weakest case.
What do NAD+ programs say about HSA and FSA?
| Program | What its page says (read 5 Nov 2026) | Source |
|---|---|---|
| Hone Health | Labels its biomarker test "FSA/HSA eligible"; says nothing of the same for NAD+ on the page we read | honehealth.com |
| System Labs | "No. System is entirely cash-pay. No insurance needed"; no HSA or FSA statement | systemlabs.com |
| Embody | "No insurance required"; no HSA or FSA statement | nad.joinem.co |
| RNK Health | No statement on HSA, FSA or insurance on the page we read | myrnk.com |
None of the four pages we read says its NAD+ is HSA or FSA eligible. Ask the program in writing before you pay.

Does insurance cover NAD+?
Not in the programs we read. They sell NAD+ as a cash service with the visit, medication and shipping bundled. For the general rules across peptides, see does insurance cover peptides.
How to check before you pay
- Ask your plan administrator whether a prescribed, compounded medication from a licensed pharmacy is an eligible expense, and whether they want a letter of medical necessity.
- Ask the program for an itemized receipt naming the prescription and the pharmacy.
- Keep records. The account holder must show the spending was eligible.
- Compare cash prices on the cost page and our best programs.
Memberships can mix parts. Hone Health prices a membership fee plus the medication on its NAD+ page; a plan may treat each part differently, so ask for the split.
When is the cash route still a fair deal?
Often. A cash program bundles the clinician visit, medication and shipping into one monthly price, with no claim paperwork and no prior authorization. If your HSA or FSA administrator does approve the expense, you simply pay with the card or submit the itemized receipt. If not, you have lost nothing but the tax advantage. The practical order is to check eligibility first, then compare prices, then enroll.
What does NAD+ cost without insurance?
Today's lowest listed NAD+ plan is $89 a month (bmiMD, NAD+ injection, 12 months, 12-month subscription). Month-to-month plans cost more than prepaid ones at the same programs. Every program and price is on the live price table.
FAQ
Can you use an HSA or FSA for NAD+ injections?
Possibly, if a clinician prescribes them for a diagnosed condition and your administrator accepts the expense. The IRS excludes expenses merely beneficial to general health. None of the NAD+ pages we read promises eligibility, so confirm with your plan first.
Does insurance cover NAD+ injections?
Not in the programs we read. System Labs says it is entirely cash-pay, and Embody says no insurance is required. Compounded NAD+ is not FDA-approved, so a plan has little reason to reimburse it. Call member services before assuming either way.
Is NAD+ HSA or FSA eligible?
There is no IRS list of eligible drugs, and none naming NAD+ that we found. Eligibility follows the prescription and diagnosis test in Publication 502. Hone Health labels only its biomarker test as eligible, not its NAD+ product.
Do I need a letter of medical necessity for NAD+?
Some plan administrators ask for one for any unusual expense. Publication 502 does not mention the letter for drugs in the text we read. Ask your administrator, and ask your prescriber whether they will write one for a diagnosed condition.
Are NAD+ injections FDA-approved?
No. Compounded NAD+ is not an FDA-approved product. It is in FDA's Category 1 of bulk drug substances under evaluation. Programs obtain it from state-licensed pharmacies on a prescription.
What is the cheapest way to pay for NAD+?
Compare the plan length first: prepaid 6- and 12-month plans usually cost less per month than month-to-month. See current prices on our cost page before choosing.
Bottom line
Plan on paying cash, and treat HSA or FSA use as something to confirm in writing. Compare programs on the cost page or the best programs.
